Data Subject Application Form under the PDPL

DATA SUBJECT APPLICATION FORM UNDER THE PERSONAL DATA PROTECTION LAW

Pursuant to the Personal Data Protection Law No. 6698 (“PDPL”), personal data owners defined as the data subject (“Applicant”) are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the PDPL.

In accordance with Article 11 of the PDPL, by applying to Volta Motor Sanayi ve Ticaret A.Ş. (“Volta Motor” and/or “Data Controller”);

1. You have the right to learn whether your personal data is being processed,

2. Request information if your personal data has been processed,

3. Learn the purpose of processing your personal data and whether they are used in accordance with their purpose,

4. Know the third parties to whom your personal data is transferred domestically or abroad,

5. Request the correction of your personal data in case it is processed incompletely or incorrectly,

6. Request the deletion or destruction of your personal data in the event that the reasons requiring the processing of your personal data are eliminated, to be evaluated within the principles of purpose, duration, and legitimacy,

7. Request that third parties to whom your personal data has been transferred be notified of the correction, deletion, or destruction of your personal data,

8. Object to an unfavorable outcome resulting from the analysis of your processed personal data exclusively through automated systems,

9. Request compensation for damages in case you suffer loss due to the unlawful processing of your personal data.

You may exercise your rights by filling out this form and submitting it to the Data Controller using one of the methods described below, in accordance with Article 13 of the PDPL and Article 5 of the Communiqué on the Procedures and Principles of Application to the Data Controller:

I accept and declare that my application to your company shall be evaluated in accordance with Article 13 of the PDPL and that I be informed via a method you deem appropriate, that the applications may be charged in accordance with Article 6, Clause 5 and Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, and that Volta Motor has the right to request additional documents and information (national identity card, copy of driver's license, other documents serving as official identification, etc.) for identity and authorization verification to ensure the security of personal data.
